In the Forms section, providers can assign specific forms to patients for completion, track their progress, and even complete the forms on behalf of patients if necessary. Here’s a detailed guide:
Steps to Request Forms from Patients
Navigate to the Patients section from the main menu and select the desired patient.
Open the Forms tab in the patient’s profile.
Click on the + New Form button to assign a form.
Select the appropriate form template from the dropdown menu.
Configure the form:
Frequency: Choose whether the form is a one-time request or recurring.
Reminder Date: Set a date to remind the patient to complete the form.
Click Add & Send to finalize the request.
Managing Form Requests
All requested forms are displayed in the Forms tab with their statuses:
Pending: Awaiting patient completion.
Completed: Submitted by the patient or completed by the provider.
For each form, the provider can:
View Form: Open and review the form details.
Complete Form: Fill out the form directly if needed and mark it as complete.
Remove Form: Cancel the request if the form is no longer required.
